If you have a Need Request for your ClearShare 1000, ClearShare 2500, or ClearShare 5000 plan, please use this form instead.

Important update

Savvos app update

Starting July 1, 2026, all holistic reimbursement requests must be submitted through the Savvos app.

  1. Go to savvos.com/clearwater/login.
  2. Log in with your plan email address.
  3. Click the "Billing" page to make sure your bank account and routing number are correct.
    We are no longer able to support check reimbursements. We apologize for any inconvenience.
  4. To start a reimbursement request, click "Start an in-person visit," then select the wellness reimbursement option under the Holistic category and complete the required information.
